In order to register, you have to fill out, sign, and return the following forms via email to principal@mtpleasantsdaschool.org:
- Student Enrollment
- Computer Acceptable Use Policy
- Special Education Awareness
- Health Appraisal
- Consent to Treat
- Over the Counter Medication
- Photo Release
- Financial Contract
- A copy of the student’s birth certificate needs to be kept on file. If the parent is uncomfortable, use the verification form below.
Optional Forms (forms needed for special circumstances):
- Birth Certificate Verification – A copy of the student’s birth certificate needs to be kept on file. If the parent is uncomfortable, use the verification form.
- Driver Information (If you will be driving other students to and from our school)
- Guest Student Application (for homeschoolers and other types of students that are not officially enrolled in our school but want to participate in school activities or trips)
